I apologize if these are repeat questions:
- Is there a way to create a list of favorite ICD-10 codes that can be shared between a provider group?
- How do your providers search for and select their ICD-10 codes? Our ophthalmology group finds it cumbersome and I was curious how other providers have simplified it. (They are using Smart Search.)
- Does eCW have some way of warning a provider that they’re using a “Manifestation Code” incorrectly?

Thanks for any input you can share

CMS Required Coding on progress notes?

Please hang tight on the explanation of this question. Coding is definitely not my wheelhouse.

Does CMS have required CPT codes that must be coded on certain visit types of progress notes? We’re in an ACO if that matters.

Example
Preventive Care and Screening: Screening for Depression and Follow-Up Plan -
A patient is positive for depression. Coder must input a code that 1. a depression screening was done. 2. it was positive 3. A followup was done. So three codes just for the depression screening if positive?

In researching our database I’ve found that none of these codes have been input for positive screens. I found a handful in the past couple years and even those not all three codes were used. I’m hoping it’s optional?

Example 2
AWV
The initial visit requires the initial visit code (Gxxx) and the subsequent visit requires (Gxxxx). I’ve found in almost all 300+ AWV’s in the past year(s) that the subsequent visit code was used for the initial AWV.

Clear as mud?

ICD 10 codes not mapped to ABNs?

It seems like the ABN mappings arent up to date with certain ICD 10 codes, although I am shocked i havent heard about this before.

Some background:

I was configuring an Advance Beneficiary notice for a lab vitamin D 1, 25 hydroxy (CPT code 82652) when I noticed that the ABN was still appearing when ordering the lab with the Approved Medicare diagnosis.

I contacted support, and after some time the tech showed me that these mappings are linked to the content distribution, for which there was an item for ABN rules from 2015, but no new downloads were available.

When looking at the database with the tech, some of the labs designated as limited coverage by medicare were linked only to ICD9 codes, including the lab I was trying to setup.

I went back and ordered the lab with the corresponding ICD-9 code, and the ABN did not prompt, showing that the tech told me this was in-fact correct about the content distribution being responsible for the ABN mappings.

I am wondering what people have been doing to deal with this issue? if an ABN is required to be given, and we dont have control over the mappings, which aren’t configured correctly, what other options are there?

Thanks for any recommendations.

ICD9 flag

We are still getting records (in report, data extracts) that show ICD9 flag. These records are associated with all visit status, namely CHK, CANC, N/S, PEN, R/S or blank. They span across both clinics and all providers.
What we notice is in these records providers have not selected any diagnosis, which is a good thing. They may have inserted some narrative, etc.
What is not clear to us why ICD9 flag still get assigned unless providers select a diagnosis?
Here is our guesses:
ICD9 codes still exist in the system as a default?

I appreciate your insight. thank you. Kumar

Hello!

I am a medical scribe for Neighborhood Healthcare who was recently informed of new diagnosis regulations for patients with diabetes mellitus type 2 + hyperlipidemia. Specifically, Molina Healthcare is requiring an appropriate Diabetes HCC. Moreover, I have been having trouble finding “Diabetes Type 2 with hyperlipidemia and renal manifestations”. Is eClinicalWorks in the process of adding this, or is there a way to add this diagnosis myself into the program? Thank you.

Best,

ICD 10 and BMI % for 18-20 year olds

ICD 10 labels 18-20 year old patients as pediatric when coding for BMI as does CDC guidelines. Some payers are rejecting claims if we don’t use the pediatric BMI% codes for these patients. eCW only calculates and auto-populates BMI % in the vitals on ages 2-19. I don’t see anywhere to change this as a setting. Any word on if eCW will change the auto-calculate/populate function to align with ICD 10 coding with any upcoming updates? It should calculate BMI% through age 20 yrs 11.99 mos.

ICD-10 and Drug/Disease Interaction not working

Anyone else have this issue? If you convert your Problem List to ICD-10 and remove the ICD-9s, any prior Drug/Disease Interactions will not be found. If you have a post-10/1 new patient and populate the Problem List with ICD-10s, no Interactions will be found. If you convert your ICD-10s in the PL back to ICD-9s, the Drug/Disease Interaction will work.

We submitted a ticket this week and eCW poked around for a couple hours and the problem still exists.
