Has anyone activated the bright futures tool kit for version 12? How do the templates look? CHADIS has integrated with Bright Futures as well just comparing the two.

Developmental Screening Tools

Good morning all,

I have been asked to create templates within eCW or perhaps a questionnaire to mirror the ASQ developmental screening tool. It is my understanding these screenings are performed through 60 months of age. Upon reviewing the ASQ forms they are quite lengthy and multiples pages. Our Clinic is relatively new to pediatrics and I am not that familiar with these developmental screening tools. How is everyone collecting this data? Is there a smart form available? Do you create WCC visits incorporating the appropriate questions?

Hello,
We are using the “Immunization Admin Code Setup” for vaccine coding, and it works except for Vaxelis. For some reason, it will drop the 5 units of 90461. Does anyone know where the setting is to fix this?

School Health Forms

The current school form/immunization record that was built out for us is inadequate. I have put in a ticket to have it fixed but the response I got was not helpful and my problem persists. Is there anyone who has a great form, particularly in Massachusetts, that might be able to help me with this?

Specialty forms

When we do a specialty form we save it and it shows up in patient documents, this is not happening anymore! Has anyone else had this problem?

Changes to Specialty Forms hard to read

Is anyone having issue with Specialty Forms : 1) retaining signatures if accessed from the progress note 2)document being split into 2 pages in an unlikely place if accessed via patient documents (which is the only way you can get the signature on the saved document) and 3) the completed “check” boxes unreadable in a completed form (used to be bold, dark selections, now our outside entities cannot determine ‘what’ was selected because the update is very light grey, no longer bold?)

If you have any of these issues - what have you done to adjust your process?

How do you document a history of chicken pox so that it becomes part of an immunization record?

Primary Proxy user for multiple patients

We are not yet live with the Proxy but have been able to enable it in a test environment. I am unclear about the primary proxy user account for multiple patients and how that links. For example if a parent is not a patient but has three children who are patients, how will those accounts link?

Asthma Action Plan

I see some posts about Asthma Action Plan about using custom forms. Has anyone figured out a good way to do this rather than spending money on custom forms?
