So I finally get all the bugs worked out to begin sending true 5010 claims only to find out that eCW has a bug that severely cripples me when billing vaccines. I went into all of our vaccine CPT codes & made sure to add a “1” to the National Drug Unit box. However, when the CPT code is added to a claim, that tiny piece of information isn’t carrying forward.

That now means that EVERY time I have a claim with a vaccine on it (which in Pediatrics is at least 50% of our claims), I now have to MANUALLY add the “1” on the Additional Information tab for EACH vaccine. Most well child visits have at least four vaccines on them.

Just talked to eCW support about this & was told there is currently no fix along with the standard line about how it should be fixed with the next update that will come out sometime in the indefinite future. Seems like ALL the known 5010 requirements should have been covered with the 5010 patch!!!

We’ve just switched to Gateway and are trying ERAs again. We tried several years ago and had too much trouble with BCBS so we went back to the old-fashioned way. Still seem to have some of the old problems with BCBS.

eCW/Gateway combo ERAs can’t handle the Michigan BCBS extortion fee - I mean the contribution to the reward pool - that is held back. Has anyone managed to get this fixed?

My hope is that one of the big hospital systems is using Gateway and has enough clout with BCBS of MI or Gateway or Ecw to have them work out a fix. Or even a very persistent small practice

Normal carriers just lower the fee schedule and give you extra money if you meet their criteria. That;s pretty straightforward, honest and easy to post. BCBS has their “allowed” amount but then they take a chunk out of it for redistribution to practices that meet their criteria. So the “allowed” amount from the ERA isn’t really the true allowed amount, and the “extortion fee” isn’t adjusted off anywhere.

I know there have been a bunch of practices from Northern Michigan that have joined in the last few years. Anyone vaguely close to Petoskey that either has a users group going or would be interested in meeting occasionally please send me a PM.

Just a word of warning for one and all. We had not installed 5010 since the deadline was extended to March. Unfortunately, our insurance carriers have switched already and most of our claims are being kicked back for not being 5010 compliant. Needless to say, we ran the update this weekend!
